We provide community-based support to help individuals and families find stability and move forward.
MOBILE CRISIS RESPONSE

We partner with the 988 Suicide and Crisis Lifeline and our Regional Crisis Hub to provide immediate, in-person support to individuals experiencing a behavioral health crisis. If you or someone you know is experiencing a behavioral health crisis please call or text 988.
STABILIZATION

Community Stabilization provides short-term, intensive support to individuals ( Children 8+ years, Adolescents, & Adults) who are experiencing a behavioral health crisis or recent disruption in functioning. Services are delivered in the home and community to help individuals regain stability, strengthen coping skills, and safely transition to ongoing care.
Services May Include:

Ongoing assessment of safety and stability

Development and implementation of crisis and safety plans

Support with emotional regulation and coping strategies

Assistance with daily functioning and problem-solving
-
Coordination with providers, family members, and community resources
-
Linkage to outpatient therapy, psychiatry, and other ongoing supports
Who Should Refer?
Referrals for Community Stabilization are accepted from emergency departments, inpatient discharge planners, mobile crisis teams, crisis receiving centers, Community Services Boards (CSBs), group homes, and community agencies.
MENTAL HEALTH SKILL BUILDING

Mental Health Skill Building Services provide individualized, community-based support to adults who need assistance developing the skills necessary to manage their mental health and maintain independence. Services focus on strengthening daily living skills, improving emotional regulation, and increasing stability in home and community settings. Our team works one-on-one with individuals to build practical strategies that support long-term functioning and reduce the risk of crisis or hospitalization. Progress is monitored through ongoing assessment of daily functioning and independence, allowing services to be tailored and adjusted based on each individual’s growth and needs.
Services May Include:

Medication education and adherence support

Building coping skills and emotional regulation strategies

Improving organization, time management, and problem-solving

Strengthening communication and interpersonal skills

Support with accessing community resources and services
Who Should Refer?
Referrals for Mental Health Skill Building Services are accepted from Community Services Boards (CSBs), therapists, psychiatrists, hospitals, primary care providers, and community agencies.
Individuals and families may also reach out directly. All referrals are reviewed by our clinical team to determine eligibility and medical necessity for services.
YOUNG ADULT LIFE SKILLS & SUPPORT

Young Adult Life Skills & Support services provide individualized, community-based assistance to youth and young adults (ages 16–21) who need support developing the practical skills necessary for independence and successful transition into adulthood. Services are designed for individuals involved with or eligible for CSA funding who may be experiencing challenges with daily functioning, decision-making, or maintaining stability across home, school, or work environments.
Our team works alongside youth in real-life settings to build essential life skills, strengthen emotional and behavioral functioning, and increase confidence in managing everyday responsibilities.

Daily living skills (cooking, cleaning, hygiene, and home organization)

Budgeting, money management, and paying bills

Who This Service Is For?
Youth and young adults who:

Are at risk of out-of-home placement or require additional support to maintain stability

Need support building independent living skills

Are preparing for adulthood, employment, or independent living

Would benefit from structured, skill-based support in the community
Who should refer?
Referrals are accepted through local CSA partners, including DSS case managers, school personnel, court service units, and behavioral health providers.
Parents and caregivers may also reach out directly, and our team can assist with connecting to the appropriate CSA referral process for service approval.
PARENT SUPPORT & GUIDANCE SERVICES

Parent Support & Guidance Services provide individualized, in-home and community-based support to parents and caregivers who need assistance managing behavioral, emotional, or developmental challenges with their child or adolescent.
This service is designed for families involved with or eligible for CSA funding, where additional support is needed to strengthen the home environment and promote stability.
Our team partners with caregivers to build confidence, strengthen parenting strategies, and improve communication—helping families navigate challenges while supporting positive outcomes for their child.
Who This Service Is For
Parents and caregivers who:

Are experiencing challenges managing their child’s behavior or emotional needs

Need additional support to maintain stability in the home

Are involved in the CSA process or at risk of higher-level interventions (e.g., out-of-home placement)
-
Would benefit from structured, hands-on guidance in real-life situations
